I have a cousin who was diagnosed 5 months ago with end-stage liver cancer. The cancer has progressed at a rapid pace, and as a result, he is in and out of consciousness. When he is conscious, he seems to be quite delirious and experiencing a significant amount of pain. His wife, who makes all his decisions, believes in natural and homeopathic remedies, and strongly disagrees with the medical team’s treatment plan to manage his pain with opioids. Despite her disapproval, the team medicates him anyway. Is this appropriate?
